If leaving a second vine, when should it be left? first one? second? up 18inches(one quote in the book). I guess because you prune off first 15inches later? I know there are lots of different opinions on this.

The first sucker vine below th first cluster of tomatoes on the main vine should be allowed to become the second main vine. All othe suckers should be pruned off after one or two leaves have developed. Prune them after those leaves are on. Train the first sucker to become the second main vine. This second main vine will allow for more blossoms and perhaps the megabloom you are looking for.

OK, after letting one or two leaves develope though, do you remove the whole sucker or prune just above those one or two leaves that you left?

So do you let all suckers grow until the first cluster shows, then remove all suckers except the one directly below the cluster? It seems to me that there would be a few suckers showing up before the first cluster?? Or am I wrong, again?

You can let the suckers all grow until you see the first fruit cluster. Marv

Picture a Bart Simpson slingshot, articulated at 45 degrees. There's your second main. Each main will produce clusters of it's own, just pinch off any suckers that appear on either one.
